Community Health Assessment & Improvement

Community Health Assessment and Improvement (CHAI) is a process that improves the health of the community through a partnership between the Health Department and community representatives. The role of the Health Department is to carry out assessment and planning activities by assisting the community with the following services:

Analyzing the health of the community
Determining priority community health needs
Identifying resources to address these needs
Developing plans and policies to address priority health needs

Information gathered through the CHAI program is useful for the Health Department and for community planning purposes.

In addition to the community initiatives, the Health Dept engages in assessment activities as part of the core function as public health agency. Data is gathered and analyzed on a regular basis to monitor the health status of the community and guide program planning and CHAI initiatives. Michigan Department of Community Health provides morbidity, mortality, and communicable disease data for all areas of Michigan.

Capital Area Community Voices (CACVoices) provides health data for the Clinton, Eaton and Ingham county area.
