Child Care/Preschool Required Immunizations

Age of Child-15 months through 4 years
DtaP or DTP 4 doses
H. influenzae type b (Hib) 1 dose after 15 months or completed series earlier
Polio 3 doses
MMR 1 dose at 12 months or older
Hepatitis B 3 doses
Varicella (chickenpox) 1 dose at 12 months or older
OR
Reliable history of disease
OR
Current lab immunity

 

School Entry Requirements

Age of Child-4 years through 6 years
DtaP or DTP 4 doses (one dose must be greater than 4 years)
Polio 3 doses (one dose must be greater than 4 years)
MMR 2 doses
Hepatitis B 3 doses
Varicella (chickenpox) 1 dose after 12 months or older
OR
Reliable history of disease
OR
Current lab immunity
Age of Child-7 years through 18 years
DtaP, DTP or Td 4 doses (must have 1 dose within last 10 years) OR 3 doses if Td series started after age 7
Polio 3 doses
MMR 2 doses
Hepatitis B 3 doses
Varicella (chickenpox) 1 dose if given prior 13th birthday
OR
Current lab immunity
OR 2 doses if given after 13th birthday
